This is a request to initiate a natural gas service line. An Okaloosa Gas Customer Service Representative will contact you to finalize any costs, natural gas load requirements, tentative service line schedule and a service line agreement.

Important: We must receive your acceptance of the natural gas service line agreement before we can schedule the job. Please fill out the form below to the best of your ability. We will have a representative contact you for pricing and scheduling within 24-hours of our normal operating hours.
